Manners Program Private Lesson Training Agreement

This agreement must be signed before starting private lessons with trainer.

Scope of Services:

• Private lessons will be conducted by a member of the training staff of Lizzi and Rocco’s. Availability of lesson dates and times are determined & filled by training staff. While a recommended duration for each training session will be provided, adjustment can be made as needed. To ensure the best outcome for your dog and yourself, the training staff may reschedule a lesson if they believe that either party is not ready for it at the scheduled time.

Session Availability

• An offered lesson spot will be held for 48 hours or 36 hours before the start time of the offered session, whichever comes first. If no response is received within this time, the spot may be offered to other clients.

Payment Policy: 

• Full payment is required at the time of service unless other arrangements have been set up with the trainer. 
• For package options, payment must be made before the first lesson that counts toward the package. If a payment plan is agreed upon with the trainer, appropriate paperwork must be signed to proceed.
• If the client cancels the package after the first session, no refund will be provided for unused sessions.

Package Expiration

Packages will expire after the following periods:

• 2-hour package: expires after 2 months
• 4-hour/6-hour package: expires after 4 months
• 8-hour/10-hour package: expires after 6 months

Cancellation Policy

• Cancellations must be made with at least 24 hours' notice. Cancellations made with less than 24 hours’ notice will be charged 50% of the session fee or will count as half the time of the scheduled session from your package.
• After three late cancellation instances, may result in future scheduling limitations, potential termination, or being placed on the waitlist. A refund will not be offered

No-Show Policy

• Failure to attend a session without prior notice will result in being charged the full session fee or a full session deducted from your package.

• After three no show instances, no further training services may be offered. A refund will not be offered.

Late Arrival Policy

• If you arrive more than 5 minutes late for a 30-minute session or 15 minutes late for a 1-hour session, the session will be rescheduled. The session will be treated as a late cancellation and charged accordingly.

Termination of Services

Training services may be terminated under the following conditions:

• Non-payment: A 5-day grace period will be given to make overdue payments. If payment is not made by 10 days, written notice of potential termination will be sent. If payment is not received by 15 days overdue, services may be terminated. The client will be responsible for all outstanding fees up to the termination date.

• Lack of communication: If there is no communication within 5 days of contact, a notice of potential termination will be sent. If no response is received within an additional 4 days, services may be terminated. The client will be responsible for fees up to the termination date.

• Non-compliance with approved training methods: Non-compliance with training methods may result in termination. All outstanding balances must be settled upon termination, and may affect future services.

Store Cancellations

• If a trainer must cancel an individual session, it will not count against the client’s package or scheduled sessions. Additionally, If the training staff or store needs to cancel any remaining sessions, you will be fully refunded for any unused sessions.

Required Supplies

• Clients must provide all necessary supplies for training sessions, including treats, rewards, a water bowl, and other items as required.

Clean up: 

• To ensure the health and safety of everyone, all owners must clean up after their animals promptly. Cleaning supplies and waste receptacles will be provided by staff.

Training Methods:

• Owners must follow the trainer’s fear free and positive directed training methods. Aversive tools and equipment including, but not limited to, prong collars, choke collars, shock collars, are strictly prohibited. Continued use of these tools after being addressed may result in termination of training services. All payments are non-refundable in accordance with our cancellation policy.
• Since training will be done using positive methods, owners are expected to bring rewards for the dogs including treats, toys, etc. (anything the dog finds rewarding).

Lifestyle Adjustments

• Clients must be open to making necessary adjustments to lifestyle or routine based on recommendations from the trainer to support their dog’s training progress.

Video & Photo Release: 

Videos and photos may be taken during any training event, as appropriate. By participating, I consent to these being published by Lizzi & Rocco’s on their website, social media, and/or for marketing. This includes images of my dog, myself, and/or anyone accompanying me to these classes or events. All videos and photos are the property of Lizzi & Rocco’s Natural Pet Market.

Liability:

I do hereby waive and release all employees & customers of Icaudi LLC d/b/a Lizzi & Rocco’s Natural Pet Market from any and all liabilities of any nature. I am fully responsible for the actions of my dog and/or any persons accompanying me to training lessons. Including, but not limited to, before, during, and after the training lessons while on Lizzi & Rocco’s and/or Academy Village property. I understand the staff and customers of Lizzi and Rocco’s will never be responsible for the actions of my dog, myself, or any persons accompanying me to the training lesson. 

I understand that participation in training does come with risk to my dog, me, and any persons accompanying me to class while on all of Lizzi & Rocco’s and/or Academy Village property. 

I agree to allow the staff to discuss details of the dog’s behavioral history, home life, relationship with owner and any other information deemed necessary with a veterinarian, animal professionals, family members, etc. as needed.

I will take full responsibility for my dog any time they are on the store premises and previously stated adjacent properties. I waive and release staff, other dog owners along with their dogs from any liability for any action that results in injury or damage which my dog, myself, or any persons accompanying me may experience. This includes any injury or damage occurring before, during, and after any training class or training event. I also waive the right from claims by me and/or any other person that attends training with my dog resulting from any dog, including my own.

I am bound to this agreement, along with all agents discussed within this agreement and included herein as spouse, children, executors, and all other agents. I understand this agreement supersedes all prior discussions, representations, warranties, and agreements of the parties discussed. I further understand, without limiting the generality of the foregoing, I acknowledge that the training staff and Lizzi & Rocco’s has not represented, promised, guaranteed, or warranted that my dog will never bite, be dangerous or vicious in the future, will not display behavioral problems, or that the results of the training will last for any particular amount of time.
